1. Does Delta Dental sell individual policies?
Yes, Delta Dental of Arizona does offer individual and family plans, click here or select Individual and Family Plans from the main menu.
2. What happens if I visit a dentist who isn't part of the network?
Your Delta Dental benefits also cover services performed by dentists who don't participate in our network. However, work done by non-participating dentists is covered at a lower benefit level and may result in greater out-of-pocket costs. back to top
3. What happens if I visit an out-of-state dentist?
If the dentist you see is outside Arizona, yet participates with that state's Delta Dental, benefits will be based on Delta Dental of Arizona's allowable fees.
4. If you are a Delta Dental National Coverage subscriber:
If the dentist you see is outside of Arizona and not a Delta National Coverage Participating Dentist, benefits will be based upon Delta Dental's non-participating fee allowance.
5. Can my dentist join the network?
If you'd like us to invite your dentist to join Delta Dental's network, contact our Professional Services Department at (602) 938-3131 in the Phoenix metro area or toll free at (800) 352-6132, ext. 6. We'll get in touch with the dentist. If he or she decides to participate, and meets Delta Dental's credential and geographical selection criteria, we'll enroll him or her in our network.

7. My benefits come through COBRA. Who handles my plan?
Please contact the employer through which you receive your benefits. Delta Dental complies with COBRA rules and regulations and accepts existing COBRA participants. We do not, however, assume the employer's responsibility for notifying, billing and administering COBRA legislation.
8. How are subscribers' questions handled? Will you provide a toll-free line for this purpose?
Our Customer Service Department is happy to answer any questions and can be reached in the Phoenix area at 938-3131, or statewide toll-free at (800) 352-6132, ext. 2. This department is located at our corporate headquarters in Phoenix, Arizona, and is staffed with knowledgeable, experienced representatives. For the convenience of our subscribers, our toll-free line is open Monday through Thursday from 7:45 a.m. to 5:00 p.m., and on Friday from 7:45 a.m. until 4:30 p.m. (Mountain Time).
9. What is “balance billing”, and how do you prevent a dentist from billing the patient any amount in excess of the “Amount Allowed”?
Participating Dentists agree to accept payment based on the Contract Amount Allowed or Submitted Amount, whichever is less. If the Amount Allowed for a particular covered service is lower than the dentist's Submitted Amount, he or she agrees not to charge the difference to the subscriber.
For example: Dr. X charges $50.00 for a particular covered service. Delta Dental's Contract Amount Allowed is $40.00. When Dr. X performs the service on a Delta Dental patient, his office then files a claim. Delta Dental sends him a check for $40.00 (less any co-pay or deductible as per the group's contract), and the service is considered paid in full. Per his contract agreement, Dr. X may not "balance bill" the patient for the remaining $10.00. This feature can add up to significant savings for our subscribers!
10. Can I receive dental treatment even when I am out of the country?
