  1. What exactly is the Direct Deposit Program?
    Direct Deposit is also known as Electronic Funds Transfer (EFT). It is the transfer of funds from the Delta Dental of Arizona account to a participating providers checking or savings accounts at a participating bank, savings and loan association or credit union.   back to top

  2. Do I have to participate in the Direct Payroll Deposit Program?
    No; participation is optional. However, there are many benefits to participation including:

    Increase staff productivity – We recognize that time is short and gas prices are high. Time traveling and/or standing in line at he bank is time better spent in your dental practice. Direct deposit allows your staff to stay productive in your office.

    Reduce the risk of theft or fraud – Whether it is mail theft or embezzlement, direct deposit provides greater assurance/accountability for funds. Arizona ranks first in mail theft and surveys show that 35% of your colleagues have been the victims of embezzling and 17% of them more than once.

    Predictable cash flow – Know exactly when your funds from Delta Dental of Arizona will be in your bank account. No longer worry about mail delays or trying to make it to the bank in time for a deposit.

    More manageable Explanations of Payment (only available on the website) – Now you can sort the information by subscriber name or number, by dentist and subscriber name or by dentist and subscriber number.   back to top

  3. Are there any costs to my participating in Direct Deposit?
    No; the program is offered as a free service to DDAZ participating providers.   back to top

  4. How can I be sure the information I provide is secure and will be kept private?
    Your information is closely guarded and handled the same way by DDAZ associates as protected health information under HIPAA.   back to top

  5. When will my money be available at the bank?
    Your money will be deposited every Wednesday unless it is a banking holiday.   back to top

  6. How will I know the funds have been deposited?
    You will have identified whether you wanted to be notified by fax or by e-mail. Each week a notification will be sent when the funds have been deposited. You can then review the Explanation of Payment through www.deltadentalaz.com or that has been faxed to you.   back to top

  7. Can I have my reimbursement deposited into multiple accounts?
    Not at this time.   back to top

  8. How do I stop direct deposit?
    You must notify Delta Dental of Arizona IN WRITING at least two weeks prior to the effective date, that you wish to stop the direct deposit and the date on which this request its to be effective. Delta Dental will then remove you from the direct deposit system and issue you a ‘live’ check   back to top

  9. What if I close or change my bank account for Direct Deposit?
    Simply complete another Deposit Authorization form. Remember to allow 10 business days for processing the change.   back to top

  10. Can I join Direct Deposit at a later date?
    You may participate at any time in the future by completing the Deposit Authorization form.   back to top

  11. How long does it take to start or change the Direct Deposit of my pay?
    Sign up is accomplished by completing a Deposit Authorization form. You should allow about 10 business days before your first direct deposit. The verification process may include a zero dollar test transaction - called a “prenote” during this 10 day period. In the meantime you will receive a paper check   back to top

  12. What is the earliest date I can make a cash withdrawal on the amount of the Direct Deposit?
    The same day as the Direct Deposit is made to your bank.   back to top

  13. What if my bank did not receive my Direct Deposit or the amount does not agree with the amount indicated on my Payroll Deposit Advice?
    This is highly unlikely; however, if it should occur, contact your Professional Relations Department at 1.866.746.1834 ext. 3. They will initiate the appropriate action to correct the situation.   back to top

  15. How will I know if my bank is a member of the Direct Deposit system?
    Most banks, credit unions or savings and loans are members of the Direct Deposit System. If you have a question, you should call your bank and ask if it is a member of the Automated Clearing House (ACH) who participates in Direct Deposit of Pay.   back to top

  16. The enrollment form asks for my checking or savings account number and my bank's transit number. How do I find that information?
    Checking and savings account numbers are normally noted at the bottom of checks and deposit slips as well as the nine digit transit number. For a sample click here.   back to top

  17. Why do you need a voided check or deposit slip?
    This additional documentation eliminates confusion or errors on properly recording your bank’s transit number. The voided check and/or savings deposit slip should be attached to or copied and faxed along with the enrollment form. The account and transit numbers should be verified with your financial institution to determine that these are the correct numbers for ACH transactions.   back to top
