Eligibility

Did you know that Delta Dental is the oldest, largest, and leading dental carrier in the nation? Everyone deserves a healthy smile... it's what Delta Dental of Arizona is all about.

1.  Who is eligible to enroll?

You are eligible to enroll for our individual and family dental and vision insurance coverage if:

  • You are an Arizona resident and age 18 or older.
    (If you are not an Arizona resident and need Individual dental coverage, click here).
  • You are not currently a Delta Dental of Arizona group plan enrollee.
  • You are not eligible for group coverage through Delta Dental of Arizona.

Eligible dependents include:

  • Your lawful spouse
  • Unmarried children under age 19 or 23 (if a full-time student) or children of your lawful spouse, including newborn children, stepchildren, disabled children, persons under legal guardianship substantiated by a court order, legally adopted children and children placed for adoption with you in accordance with applicable state or federal law. 

Note: Verification of dependent status or full-time student status for children over age 19 may be required.

2.  I have a pre-existing condition. Can I still enroll in an Individual Plan?

Yes! Every resident of Arizona, with the exception of current Delta Dental of Arizona members, is eligible regardless dental history or pre-existing conditions. Waiting periods do apply for Type 3a & 3b services

3.  What if I decide to drop Delta Dental Individual Plan coverage?

Enrollments are for consecutive 12-month period(s) and the monthly/annual premium payment is subject to change on the anniversary date. Non-compliance with these terms voids any benefits during that enrollment period. Should a member decide to cancel the plan, a 30-day written notice is required. Once notice is received, the policy will be cancelled at the end of the month and billing will not be prorated. All individuals previously covered under the plan will not be allowed to re-enroll within the subsequent 24 months. Cancellation Request Form 

4.  Are there waiting periods?

Yes. To help keep your premiums low, there are waiting periods for Type 3a & 3b services. However, from the date you are eligible, you can start saving immediately on Type 1 & 2 services.

  • No waiting for Type 1 - (Cleanings, exams, etc)
  • No waiting for Type 2 - (Fillings, extractions, etc)
  • 12 months for Type 3a* - (Crown Repair, Root Canals, etc.)
  • 24 months for Type 3b* - (Dentures, Fixed Bridgework, etc)

If within the past 60 days you have been covered under a Delta Dental group plan, and had at least 12 months of continuous coverage under that plan, waiting periods may be waived.

5.  Important Note about Your Effective Date

When valid enrollment documentation and payment is received by Delta Dental on the 1st through the 21st of the month, coverage will be effective the first of the month immediately following. When valid enrollment documentation and payment is received by Delta Dental on the 22nd through the last day of the month, coverage will be effective the first of the second month.

For example: If your enrollment documentation/payment is received January 15, your coverage effective date is February 1. Similarly, if enrollment your documentation/payment is received January 25, your coverage effective date is March 1.
