Registering for the DDAZ Online Enrollment Application

To register for Online Enrollment, take note of the following steps.

Please use the link at the bottom of the page, after reviewing all of the instructions, to start the process.    You may wish to view or print complete instructions for reference. 

Step 1
First, log in to Delta Dental of Arizona 's Website using your existing Employer Connection information.  

Please enter your existing username and existing password for the Employer Connection.

 

Step 2
After login, request modification to your account. When requesting access to Online Enrollment, you will be presented with a screen to update your existing account.

Scroll down to verify that Online Enrollment is selected.  Click the button labeled Submit.

Step 3
Registration Confirmation.
For your protection, we will validate your data against our files and contact you as quickly as possible to confirm your request for online enrollment.
