Enrolling in Delta Dental of Arizona is as easy as 1, 2, 3…

1. Select your dental plan, you have 2 plans to choose from -- Dental Plan Comparisons  (New Rates Effective 7/1/2008)
   
   
2. Complete the enrollment form and choose how you will pay for your benefits - monthly via electronic funds transfer (EFT) from your checking or savings account or annually by check. -- Enrollment Form )   (New Rates Effective 7/1/2008)
   
   
   • If you choose the monthly EFT option, you are required to include the EFT Authorization Form and a three-month premium Down Payment with your enrollment form. -- Electronic Funds Transfer Authorization Form
   • If you choose to pay annually by check, your Total Payment must be included with your enrollment form.
     
     
3. Mail your completed enrollment form, and either your EFT authorization form with down payment or annual premium to:

   Phoenix, AZ 85080-3000
   

Note: Your dental benefits will begin the first day of the month following Delta Dental of Arizona’s receipt of your completed enrollment materials.
