Delta Dental Premier Dentists (Fee-for-Service)
Because this is the largest of Delta Dental of Arizona's networks, with more than 85% of Arizona dentists participating, it is very likely that most subscribers will find their dentists listed here. Dentists listed in this directory provide services under the Delta Dental Premier program and have agreed to:
- file claim forms for their patients
- accept payment directly from Delta
Dental
- charge patients only for their copayment (if any) and/or deductible (if any) for covered benefits
- charge his or her usual fee or Delta's Maximum Plan Allowance, whichever is less
